Painful Ejaculation
The burning sensation or pain felt by a man during ejaculation is called painful ejaculation or a painful orgasm. The pain can be felt between the genitals and anus or also in the testes or urethra. This condition is very frustrating for the man causing him to avoid sex. He may suffer from anxiety or depression due to this condition and his relation with his partner may also suffer because of painful ejaculation.

Causes of painful ejaculation

Listed below are the possible causes of painful ejaculation:

  1. STD or sexually transmitted diseases.
  2. A nerve damage in the penis or urethra
  3. Inflammation of the prostrate, urethra or testicles.
  4. Blockages in the nerves responsible for ejaculation.
  5. Pelvic pain or radiation
  6. Cancer in the prostrate
  7. Stress or depression
  8. Prostatectomy or the removal of the prostate gland also leads to painful orgasms.
  9. Some medications like contraceptive creams, anti-depressants and others may also cause painful ejaculations.
  10. Chronic non-bacterial Prostatitis.

Sometime lack of ejaculation also causes pain. Retrograde ejaculation is when the neck muscles of the bladder do not tighten properly and the sperm enters the bladder instead of ejected out through the penis. Analyzing the possible cause of painful ejaculation can only help treat painful ejaculation.

Delayed ejaculation also causes pain. In this condition, the semen enters back into the bladder during an orgasm instead of coming out of the penis. This is then flushed out by the body while urinating. Various conditions like diabetes, medications, surgery and nerve damage can cause such issues.

Painful Ejaculation Treatment

One needs to determine the cause of the painful ejaculation to treat it. If the pain is due to nerve damage in the urethra or penis, it will heal on its own in 2 years. Some medications may help if there is inflammation of the testes, urethra or the prostate.

Anti-depressants or contraceptive creams should be stopped for some time to determine the underlying causes of the painful ejaculations. The doctor might take urine or semen samples to analyze the reason. Sometimes, the reason may be chronic non-bacterial prostatitis. This itself is very difficult to diagnose but once it is, it can be cured by medicines.

A bacterium that causes urinary tract infections also causes acute prostatitis in men. This bacteria travels to the prostate via blood. Acute prostatitis is a condition in which the prostate gland gets inflamed. This small walnut shaped organ is located below the bladder in men. It secretes a fluid that nourishes the sperm. When a man ejaculates, the prostate squeezes the semen into the urethra.

Inguinal hernias cause a lot of discomfort in men. The intestines start to move towards the inguinal canal and while doing so they put a lot of pressure on the spermatic cord. This pressure results in mild discomfort to sometimes exquisite pain. This condition sometimes can be seen as the intestines bulging out at the crease of the upper or inner thigh and sometime in the scrotum.

The intestines put pressure on the nerves into the testicals and this result in testicular pain. On ejaculation, every muscle contracts in the pelvis to release sperm. This causes inflammation on the hernia.

There are two types of surgeries performed for hernia. One is the open surgery in which a local anesthetic is given. A single incision of 6-8 inches is made on the hernia. A mesh is put in the abdominal wall at the weakened spot where the hernia bulged from in order to strengthen it. Later, the skin is sealed with stitches.

The other kind of surgery is the laparoscopic or the keyhole surgery. In this 3 incisions are made instead of one. The basic aim in this surgery is to pull back the hernia into its place by the help of the inserted camera and tools. Lastly, the incisions are stitched to seal the skin.

The mesh being a foreign object causes chronic pain in some 20% patients while it heals successively in most others. Some doctors believe the pain is due to neuropathy or ischemia induced pain.

Ejaculatory pain after hernia surgery

About 10% patients experience chronic ejaculatory pain after hernia surgery. This affects their daily life. Various tests were done to find out the cause of the pain. It was concluded that the pain was due to problems of somatic origin. All the patients had sensory nerve disturbances like hypesthesia or hyperalgesia which means damage to large and small fiber nerves of that area.

The pain is caused due to a lesion in the vas deferens while surgery. The pain is of neuropathic origin. The performing surgeon should be an expert in treating inguinal hernia else it will result in ejaculatory pain after the hernia surgery.

Painful Ejaculation STD

Sexually Transmitted Diseases or STD is very common. Every year 20 million cases of STD are registered in the United Nations. Many people remain undiagnosed with STD as it has no symptoms. Untreated STDs can:

  • Increase chances of infertility.
  • Lead to cancer
  • Is very contagious and can spread to partners
  • Damage unborn baby in the womb
  • Increase risk of HIV.

Certain STDs like Chlamydia are easy to spread. There is a burning sensation while urination or ejaculation. The scrotum is full of liquid and mass. One gets a cheesy-watery like discharge from the urethra in different amounts. There might be some yellowy discharge too. A chlamydia test should be done to rule out the possibility of any STD and if found should be cured immediately. The test involves insertion of a long swab into the urethra to test it in the lab.

Other STDs like Paraphilias, Trichomoniasis, Genital Warts, Gonnorhoea , Syphillis and genital herpes also cause painful ejaculation.

Painful ejaculation can be treated and one should consult a doctor at the earliest. Delay in medical assistance will lead to several disorders in the body and will continue to mar you from enjoying a fulfilling sex life with your partner.

    I have severe thigh pain during ejaculation. I also have retrograde do to the medication I’m taking for enlarge Prostrate.

    No one can tell me why the pain (to tears) happens.

    any help ?
